Financial Assistance Programs
We are committed to supporting our community by offering financial assistance programs for qualifying players. Starting January 1st, 2025, eligible applicants can access support through the following programs:

Eligibility
To qualify, applicants must:
- Be aged 5 to 18 years.
- Meet financial criteria (e.g., net family income below $45,000).
- Each program has specific requirements and supporting documentation, so please review the forms carefully.
How to Apply
- Download the application form(s) below.
- Complete the form(s) and gather any required supporting documentation, such as financial statements or endorsements from community professionals.
- Email a clear image of the completed form(s) and supporting documents to Admin@SportifyFC.com by December 31, 2024.
Important Notes
- Applications will be processed on a first-come, first-served basis until funding is fully allocated.
- Early applications have a higher chance of approval due to limited funding.
- Ensure all documents are complete and legible to avoid delays.
